We will contact you within a couple of days to introduce you to our clinic, give you access to our patient portal, and assign appropriate forms and questionnaires. We’ll let you know if a referral is needed.
After we receive the completed forms, we will contact you to schedule an appointment.
Send any relevant records to us by email or upload them to the portal.
